Child and Teen Checkup (C&TC) Outreach

A child's health, growth, and development should be checked regularly. C&TC Outreach Program can help families find a doctor, dentist, psychologist, and transportation to receive regular Child and Teen Checkups.

Disease Prevention & Control

Public Health investigates and monitors actual or potential disease processes in the County. Communicable diseases are prevented through:

  • Adult & child immunizations
  • Communicable Disease Prevention Program
  • Emergency Preparedness Plan
  • Surveillance of the county's environment
  • Tuberculosis screening

Environmental Health

The Environmental Health Program does assessment and monitoring of environmental conditions that impact our county's health by the following methods:

  • Water testing kits
  • Radon testing kits
  • Public Health nuisance control
  • Lead abatement information

Family Home Visiting Program

The Family Home Visiting Program offers eligible families a wide range of services, information, and resources to help families provide a healthy start for their children and to become self-sufficient. A Public Health Nurse or other trained home visitors provide home visits to the family.

Follow-Along Program

The Follow-Along Program helps parents give and receive information about their child's development during infancy and early childhood by mailing questionnaires at about four month intervals.

Health Promotion

Health Promotion program consists of presentations and/or materials on health topics such as nutrition, physical activity, family health, mental health, chemical & tobacco use prevention, and others. Presentations may be for small or large groups. Individual health information and education is provided by a Health Educator or Nurse.

Long Term Care Consultation (LTCC)

The LTCC Program assists persons of any age, with long-term or chronic care needs, to make long-term care decisions through the selection of service options that meet their needs and preferences. This program is intended to prevent or delay nursing home admission. A team, made up of a Public Health Nurse and a Social Worker, visits with a person to help them assess their needs, and select service options that meet their needs and reflect their preferences.

Maternal and Child Health (MCH)

The MCH Program's goal is to help insure the optimal health of pregnant and post-partum women, infants, and special needs children. Information and assistance can be provided through MCH clinics or home visits.

Personal Care Assistance (PCA) Assessment

The PCA program provides support to eligible persons with disabilities, and the elderly. PCA services are for people who need help with day-to-day assistance in meeting their personal care needs. A Public Health Nurse conducts an assessment to determine program eligibility and needed services.

Public Health Clinics

Public Health provides screenings to individuals in apartment buildings or by appointments. The screenings available are blood pressure, foot care, diabetic care, or medication management.

Public Health Preparedness for Emergencies

Waiver Case Management (CM)

Persons with disabilities or chronic illness who need certain levels of care may qualify for the Minnesota home and community-based waiver programs. These programs available to persons who meet the eligibility criteria include: Elderly Waiver (EW), Alternative Care (AC), Community Alternative for Disabled Individuals (CADI), and Community Alternative Care (CAC). Waiver CM is conducted by a team which includes a Public Health Nurse and a Social Worker.

Women, Infants, and Children (WIC)

The WIC Program provides vouchers for nutritional foods and nutrition information to participants. This program promotes good health and nutrition for pregnant, post-partum, and breast-feeding women, infants, and children up to age five.

Other Services
