COVID-19 Closure: Community & Veterans Services Forms & Applications

Due to Governor Tim Walz declaring a peacetime emergency in response to COVID-19 and the need for community mitigation strategies to be put in place, Effective Tuesday, March 17th, 2020 at 4:30 pm, Community & Veterans Services Office is closed to the public. At this time, we will be suspending in-person meetings with clients until further notice. Staff will offer services without face-to-face contact by phone, email or mail. Our staff is available by phone Monday – Friday from 8:00 am to 4:30 pm at 320-983-8208 or 888-270-8208. Thank you in advance for your patience as we navigate this challenging situation day-by-day and determine how best to continue serving our community.

When dropping off forms or applications, please utilize our drop box. We can accept all electronic forms or applications and required verifications by email at CVS.Reception@millelacs.mn.gov or fax documents to 320-983-8306. You may apply for all programs and access documents online at https://applymn.dhs.mn.gov for SNAP, CASH, Child Care, and Emergency Assistance; https://www.mnsure.org/ for Medical Assistance and https://mn.gov/dhs/general-public/publications-forms-resources/edocs/. We will mail documents by request.
